Hillcrest Health Services is currently seeking a full-time Hospice Social Worker for our Hospice team in Omaha, NE.
The Hospice Social Worker provides psychosocial and bereavement services to all hospice patients to meet immediate needs and plan of care needs are identified.

What are my responsibilities?
- Provides psychosocial and bereavement services primarily to those hospice patients who are identified to have seven days or less to live. Specific attention to meeting the immediate care needs as related to funeral and advance directives.
- Responds to referrals, conducts hospice informationals and leads the consent process for new patients.
- Completes all charting and visits timely in accordance with state and federal regulations, as well as agency policy and procedures.
- Participates in the care planning and case conference process ensuring the involvement with patient/family and that their needs are accurately assessed and care planned.
- Provide external referral to community supports to patients, families and community facilities as requested.
What do I need to be considered?
- STAR mentality (Selfless, Tough, Accountable, Respectful).
- Bachelor's degree from accredited Social Work program required.
- At least two years of experience in Social Work, Case Management or related field.
- At least one year of social work experience in a health care setting required.
- At least one year of experience in Social Work in hospice/home care preferred.
